| Dr. Jim Swanson has been committed to making a difference in
individual’s lives through his medical practice. He elected to
establish this scholarship program in a continued effort to make
a difference in the lives of students with disabilities who are
interested in pursuing post-secondary educations. |

| Amount of Scholarship: $500 |
| |
Eligibility Criteria:
|
- Must be a graduating senior of Lawrenceburg High School;
- Must be a student with a disability as defined by the
American Disability Act (ADA); and
- Must be planning to attend an accredited college.

Evaluation Process:

Submit your completed
application to the Dearborn Community Foundation, Inc.
office no later than 4:00 pm on Monday, January 14,
2008.
- Application will be reviewed for eligibility,
completeness, and professionalism; and
- Applications will be submitted to the advisory committee
for evaluation based on student’s GPA and/or class rank,
noteworthy achievements, school activities, community
activities and financial need.
